Fox Valley Psychiatry uses Tebra as our electronic medical record and portal to provide you with efficient and secure communications.
Through the portal you can:
- Send a message and receive a reply that day
- Submit refill requests
- Review your records

Need to register for portal access?
When you scheduled your first appointment you were sent an email with a link to setup your username and password for your Tebra Patient Portal account. Look in your spam folder if you can not find the email. If you deleted that email and need another, please call (920) 882-7780 and leave the patient’s name, DOB, your preferred email, and say you want another portal setup link emailed to you.
The portal securely protects your private healthcare information. Emailing detailed healthcare information is not an option. Email is not secure enough to be HIPAA compliant.
