Login to your healthcare portal to:
- Message Your Provider
- Request a medication refill
- Review records
Messaging in the portal is the preferred and quickest method of contact. If you do not have a portal account setup yet, do so now
If you are unable to schedule an appointment online using the button to the left, then you may call: (920) 882-7780 to request an appointment. If you reach the voicemail, leave your name and DOB, phone number to call back, best times to call and in what time frame you are looking for the appointment to take place. If the client is a minor please leave the name of the caller as well as the name and DOB of the client. You will be called back within 24 hrs to schedule an appointment.
If you are seeking an urgent appointment, please state that in your call. If you are having a medical emergency call 911 or go to an emergency room. If you are in imminent danger of hurting yourself, such as planning to act on suicidal thoughts or being at risk of harm please reach out to a family member or friend or call 988 for the suicide hotline number.
